Stages of Alcoholism: How Dependence Develops
Alcoholism rarely arrives suddenly. It climbs through recognisable stages, and each stage up is harder to treat than the one before. The hopeful corollary: every stage has an exit.
The Modern Staging
- Experimentation and social use - drinking for occasion and effect, fully controlled
- Regular and risky use - drinking becomes routine; quantities creep; first consequences (hangovers at work, arguments)
- Problem drinking - drinking despite harm; hiding, guilt, failed cut-down attempts; blackouts may appear
- Dependence - tolerance high, withdrawal symptoms on stopping, drinking to feel normal
- Late-stage addiction - health damage (liver, heart, nerves, brain), life organised around alcohol; stopping without medical help is dangerous
Treatment works at every stage. Earlier is simply easier, cheaper, and kinder.
The Classical Four Stages (Mada Avastha)
Charaka Samhita (Chikitsa Sthana 24) and Sushruta Samhita (Uttara Tantra 47) describe four progressive stages of intoxication - an acute scale that also mirrors the chronic descent:
- Prathama Mada: joy, sociability, sharpened appetite and senses - Ojas still intact
- Dvitiya Mada: slurred speech, flickering memory (Smriti-bhramsha), unsteady gait, emotional swings - Ojas disturbed
- Tritiya Mada: collapse of coordination and judgement, unconsciousness (Murchha)
- Chaturtha Mada / Madatyaya: systemic collapse - stupor, delirium, entrenched addiction, Ojas destroyed
The classical lesson is in the Ojas thread: each stage deeper costs more vitality, and the fourth stage is where the texts stop offering home care and insist on physicians.
Why "Stopping Suddenly" Is Its Own Stage of Danger
At the dependence stage, the brain has rewired around alcohol. Remove it abruptly and the nervous system rebounds into overdrive - tremors, then possibly seizures and delirium tremens (confusion, hallucinations, fever, dangerous heart rhythms). Delirium tremens kills a meaningful percentage of those it strikes untreated, per NHS alcohol misuse guidance. Medically supervised detox - with sedative medication, fluids, and thiamine - makes the passage safe.
Frequently Asked Questions
How fast do the stages move? Anywhere from years to decades. Binge patterns and daily heavy use accelerate the climb; there is no fixed timetable.
Can someone skip stages? The slide can accelerate after shocks or when drinking becomes daily, but dependence still builds through escalating use - it just goes unnoticed.
What stage is "morning drinking"? That usually signals established dependence - drinking to suppress withdrawal. It warrants medical assessment, not judgement.
Do the classical stages match modern ones? The classical four describe a single session's deepening intoxication, yet their arc - intact vitality to total collapse - maps honestly onto the chronic descent. The mapping is interpretive, not exact.
