Introductory summary

Spironolactone and furosemide done safely, paracentesis with albumin, TIPS and transplant - plus the supplements and herbal water-pills to avoid or question.

Medicines and Products for Ascites

Only a handful of medicines treat ascites itself, and all of them are prescription drugs needing blood-test monitoring. Everything else - the supplement shelves, the herbal water-pills, the "liver tonics" - ranges from unproven to dangerous in a patient whose liver and kidneys are already struggling. Here is the honest map.

The Two Diuretics That Form the Backbone

  • Spironolactone - the first-line drug for cirrhotic ascites. It blocks aldosterone, the hormone making the kidneys hoard salt. Started low and increased gradually, with regular potassium and creatinine checks.
  • Furosemide - a loop diuretic added when spironolactone alone is insufficient, in a specific ratio that protects potassium balance.

Both are started, adjusted and stopped only by the treating physician. Unmonitored diuretics in cirrhosis cause kidney failure, dangerous potassium swings and encephalopathy. Never buy these over the counter, never adjust the dose yourself, and never stop them abruptly because the belly looks flatter.

Hospital Treatments Beyond Tablets

  • Large-volume paracentesis (LVP) with intravenous albumin - controlled drainage for tense or refractory ascites, with albumin replacing what drainage shifts, preventing the circulatory collapse described since Sushruta's time
  • TIPS (transjugular intrahepatic portosystemic shunt) - a radiologically placed channel that relieves portal pressure in selected refractory patients
  • Liver transplant evaluation - the definitive treatment for suitable candidates with decompensated cirrhosis
  • Cause-specific medicines: hepatitis C direct-acting antivirals (curative), hepatitis B suppressive therapy, a full course of anti-tuberculosis treatment for TB ascites, heart-failure drugs for cardiac ascites, cancer-directed therapy for malignant ascites
  • Lactulose and rifaximin when encephalopathy accompanies the ascites; antibiotics (and often long-term prophylaxis) after an SBP episode

What to Avoid or Question

  • NSAID painkillers (ibuprofen, diclofenac, nimesulide) - can precipitate kidney failure in cirrhosis; paracetamol within doctor-set limits is the usual alternative
  • Unprescribed "liver tonics" and supplements - many marketed products have no evidence, and an injured liver metabolises additives unpredictably
  • Ayurvedic products marketed for "water retention" - Punarnava-based classical formulations (Punarnavarishta, Punarnava Mandura, Jalodarari Rasa and others) exist, but none has robust trial evidence in ascites, quality and metal-content vary between manufacturers, and their diuretic action can collide with prescribed spironolactone. If you want to use them, the non-negotiable rule is: your hepatologist knows, your diuretics continue, and kidney function and potassium stay monitored.

Frequently Asked Questions

Can I buy spironolactone myself if it worked before? No. The dose that helped last month can harm this month - kidney function and potassium shift as the disease changes. Refills need the same monitoring as first prescriptions.

Are Ayurvedic diuretics a gentler alternative to spironolactone? "Gentler" usually means "weaker and unmonitored." Spironolactone's dosing is titrated against blood tests; herbal diuretics offer no such control. Adjunctive use may be reasonable with your doctor informed; substitution is not.

Why is albumin given with drainage? Removing litres of fluid shifts the circulation's volume; without albumin, blood pressure and kidney perfusion can crash (post-paracentesis circulatory dysfunction). Albumin is treatment, not a formality - do not let anyone skip it to cut costs.

Do diuretics cure ascites? They control it. Cure depends on the cause: antivirals can cure hepatitis C, abstinence can recompensate some alcoholic cirrhosis, ATT cures TB ascites, and transplant replaces the failing liver. Diuretics buy safety and comfort while those treatments work.
