Ayurvedic Definition: This condition is defined as Ojakshaya—the diminution of Ojas. Ojas is the final essence of all seven bodily tissues (Dhatus) and is responsible for biological strength (Bala) and vitality. When Ojas is lost, the body cannot resist diseases. This progression manifests clinically as Rajayakshma, a syndrome characterized by the wasting away of tissues, chronic fever, and multisystem failure.
Ayurveda categorizes the causes of immunity loss and wasting into four main categories, known as the "fourfold causes of Rajayakshma."
While HIV is a single virus, Ayurveda classifies the resulting wasting syndrome (Rajayakshma) based on the number of pathogenic factors involved:
Eka-Rupa: Manifesting with a single dominant symptom (e.g., just cough).
Sad-Rupa (Six-fold): Characterized by six major symptoms: cough, fever, side pain, hoarseness, diarrhea, and anorexia.
Tri-Rupa (Three-fold): Characterized by fever, cough, and blood in sputum.
• Difficult to Cure (Krichra/Yapya): If the disease is advanced, involving all three Doshas and severe wasting, it is considered difficult to treat.
• Factors affecting prognosis:
◦ Stool: In wasting diseases, the strength of the patient depends on the retention of feces (Mala). Excessive diarrhea is a fatal sign because the stool retains the last bit of the body's heat and substance.
◦ Digestion: If the patient retains their digestive fire (Agni) and can eat, they may survive.
• Immunodeficiency: The Ayurvedic concept of Ojakshaya (loss of Ojas) correlates directly with Acquired Immunodeficiency Syndrome (AIDS).
• Wasting Syndrome: The clinical description of Rajayakshma (fever, diarrhea, weight loss, cough) is nearly identical to HIV Wasting Syndrome and opportunistic Tuberculosis.
• Transmission: Ayurveda acknowledges diseases spread through "contact" (Prasang) and sexual intercourse (Vyavaya), classifying them under Aupasargika Roga (contagious diseases) or sexually transmitted ailments.
