Symptoms of urticaria

Introductory summary
Urticaria shows as raised, intensely itchy wheals that appear suddenly, migrate across the body and each vanishes within 24 hours; swelling of lips, tongue or throat signals dangerous angioedema.
Symptoms of urticaria
The wheal (hive) - the signature lesion:
- Raised, red or skin-coloured swellings, from a few millimetres to large patches, sometimes merging into maps and rings
- Intensely itchy - often the itch arrives before the wheal
- Appears suddenly, fully formed, in minutes
- Migrates: the defining behaviour - each individual wheal fades within 24 hours (without leaving a mark), while new ones erupt elsewhere. Hives that stay fixed in one spot for days suggest a different diagnosis.
- Can appear anywhere on the body, at any time, often worse at night
Angioedema - the deeper swelling: in about 40% of patients, swelling goes deeper than the skin - puffy lips, swollen eyelids, swollen tongue, or swelling of hands, feet or genitals. It is less itchy, more aching or tight, and takes a day or two to settle. Angioedema of the tongue or throat is dangerous.
Emergency symptoms - anaphylaxis; call 112 immediately:
- Swelling of the tongue or throat, hoarseness, difficulty swallowing
- Breathing difficulty, wheezing
- Dizziness, faintness, collapse (blood pressure dropping)
- Vomiting and severe abdominal pain accompanying the hives
- These signal a whole-body allergic reaction - this is an emergency, not a wait-and-watch situation
Symptoms that suggest it is NOT simple urticaria: individual spots lasting over 24-48 hours in the same place, burning or pain rather than itch, bruising as lesions fade, fever and joint pains - these warrant a doctor looking for other conditions (like urticarial vasculitis).
Course: acute urticaria settles within days to six weeks. Chronic urticaria (beyond six weeks) comes in waves - some patients are clear for months between episodes, others itch daily; the average duration is years, but it does eventually burn out in most people.
Remember: migrating itchy wheals that each vanish within a day are classic hives - annoying but manageable. Tongue, throat or breathing involvement is 112, every single time.
Ayurvedic view (interpretive)
Classical Sheetapitta descriptions capture the migrating, itchy, wind-like quality well. The angioedema emergency, though, belongs to adrenaline and emergency care - no traditional measure intervenes fast enough.
