How to Treat Bunions

Introductory summary
Bunion treatment step by step: shoes and insoles first, surgery as the only true correction - with honest limits.
Bunion treatment runs from shoes to surgery, in that order. The honest map - what each step does and what it cannot do.
Step 1: Conservative care (everyone starts here, many stay here)
- Wide, soft, low-heeled shoes - the foundation
- Bunion pads or sleeves over the bump; toe spacers if crowding hurts
- Cold packs for flares; paracetamol or a short NSAID course for pain (gel over the bump works well)
- Insoles (orthotics): a podiatrist's insoles can redistribute forefoot pressure and slow drift in flexible feet
- Foot exercises for mobility and strength
- Night splints: may slow progression in adolescents; comfort, not correction, in adults
- Classical comfort layer: warm oil foot massage (Padabhyanga with Pinda taila) and warm soaks - daily, gentle, pleasant
Realistic goal: a comfortable foot that walks as much as you need. This stage can hold for years - many people never need more.
Step 2: When conservative care is not enough
The signal: pain despite good shoes, shrinking walking distance, skin breakdown, or a deformity racing ahead. Now: X-ray, staging, and a surgical opinion. An injection into an inflamed bursa helps some; custom orthotics get a formal trial.
Step 3: Surgery (the only true correction)
- Over a hundred procedures exist; modern bunion surgery usually repositions the metatarsal bone (osteotomy) with screws, sometimes with joint soft-tissue balancing; severe arthritic joints may be fused
- Day-care or short stay; walking in a protective shoe typically begins early; full recovery takes weeks to a few months; swelling settles over months
- Success rates are high in experienced hands - but recurrence, stiffness and persistent aches happen, which is why surgery is for bunions that limit life, not for appearance
- Choose the surgeon by their foot-and-ankle experience, and ask: which operation, what recovery, what recurrence rate in your practice?
What the classical texts add
Their own verdict matches modern medicine: chronic joint deformity with fixed misalignment was graded yapya - manageable, not curable by medicines alone (Charaka Chikitsa 29; Madhava Nidana 23). And classical texts explicitly did NOT advocate forceful manipulation for chronic deformities - gentle oils, fomentation and pastes for comfort; intervention only for fresh injuries. Ancient honesty, modern agreement.
One-line summary
Shoes, pads, insoles, exercises and classical comfort care carry most bunions for years; when pain wins despite them, realignment surgery by an experienced foot surgeon is the definitive fix - and the only one.
