Stages of Colon Cancer

Introductory summary

Stages 0 to IV explained - what each stage means for treatment and why early detection changes everything.

Colon cancer staging runs 0 to IV and drives every treatment decision. The hopeful headline first: caught at stages 0-II, this cancer is curable in the great majority of patients.

Stage 0 (carcinoma in situ)

Abnormal cells confined to the innermost lining - not yet a true invasive cancer. Removed completely at colonoscopy or local surgery. Essentially a cure on the spot.

Stage I

Cancer grown into the colon wall's inner layers but not through them; no lymph nodes. Surgery alone usually cures - five-year survival around 90%.

Stage II

Growth through the full wall, still no lymph nodes involved. Surgery is the mainstay; chemotherapy is added for higher-risk features (certain tumor grades, obstruction, few nodes sampled). Survival remains high - roughly 70-85% at five years.

Stage III

Cancer has reached nearby lymph nodes. Surgery PLUS chemotherapy is the standard - the chemo meaningfully raises cure rates. Five-year survival roughly 50-70% depending on node count. Curable intent still.

Stage IV (metastatic)

Spread to distant organs - liver and lungs most commonly. Not usually curable, but genuinely treatable: some patients with limited liver or lung deposits are operated with cure in mind; others live years on chemotherapy, targeted drugs and (for MSI-high tumors) immunotherapy. This stage is managed as a chronic fight with real weapons, not a surrender.

The TNM behind the number

Doctors build the stage from T (how deep the tumor has grown), N (how many lymph nodes), M (whether it has spread). Scans (CT), the surgery specimen and blood CEA levels complete the picture.

The screening logic, again

These numbers are why screening matters so much: the same cancer found at stage I is a 90% cure story; at stage IV, a chronic battle. A colonoscopy moves people from the right column to the left.

One-line summary

Stage 0-I are near-certain cures, II-III remain curable with surgery and chemo, IV is treatable for years - staging is the reason early detection changes everything.
