Complications and Risks of COVID-19

Reviewed by medical experts · For informational purposes only

Introductory summary

Most COVID ends in a week or two, but its complications are real: pneumonia and oxygen dependence in the acute phase, blood clots and MIS-C rarely, and long COVID - months of fatigue, brain fog and breathlessness - in a stubborn minority. What can go wrong, who it happens to, and what helps.

Most COVID infections end cleanly. The complications worth knowing fall into three groups: the acute dangers of severe illness, the post-COVID tail, and the risks concentrated in specific organs and people. Calm knowledge, not fear.

The acute complications (severe illness)

  • Pneumonia and respiratory failure: the virus inflames the lungs - the main reason for hospitalization, oxygen and ICU care
  • Blood clots: severe COVID promotes clotting - leg clots and lung clots (pulmonary embolism) can strike during or just after illness; new one-sided leg swelling or sudden breathlessness after COVID is an emergency
  • Secondary infections: bacterial pneumonia or other infections joining during or after the viral illness
  • Heart involvement: myocarditis (heart inflammation) and rhythm problems - uncommon, mostly in severe illness
  • Multi-organ strain in critical illness: kidneys, liver and brain can be affected in ICU-level disease

Long COVID - the honest account

Symptoms persisting or appearing beyond 4-12 weeks: crushing fatigue, brain fog, breathlessness, palpitations, sleep disturbance, mood changes. It is real, recognized by medicine, and more common after severe illness - though mild cases can develop it too. There is no single test or cure; management is proper evaluation (to exclude other causes), paced rehabilitation (graded activity without boom-bust cycles), breathing retraining, and treatment of specific symptoms. Most people improve substantially over months. Anyone selling a long-COVID miracle cure is selling hope, not medicine.

For specific organs and people

  • Diabetes: COVID can worsen sugar control sharply during illness - closer monitoring, and medication adjustments by the doctor
  • Heart patients: higher risk of events during and after infection - lower threshold for review
  • Pregnancy: moderately higher risk of severe illness; infection in pregnancy warrants medical follow-up
  • Children: severe disease is rare; a rare post-infection inflammatory syndrome (MIS-C) presents with persistent fever, rash, red eyes and abdominal pain weeks after infection - hospital evaluation needed

One-line summary

COVID's acute complications - pneumonia, clots, secondary infection - drive hospital care; long COVID is real and managed with paced rehabilitation; and diabetes, heart disease, pregnancy and children each carry their own specific cautions.
