Complications and Risks of Fistula
Reviewed by medical experts · For informational purposes only
Introductory summary
Recurrent abscess, complex tracts, incontinence after surgery, recurrence, and urgent warning signs.
Anal fistula is not usually life-threatening, but it can cause lasting problems if not properly treated.
Complications of the fistula itself
- Recurrent abscesses. The tunnel can block and the infection can return again and again.
- Spread of infection into surrounding tissue (cellulitis) or, rarely, into the blood
- Chronic discharge and skin irritation, affecting hygiene and daily life
- More complex tracts. Repeated infection can branch the tract and make later surgery harder.
- Pain and reduced quality of life, including problems with sitting and intimacy
- Rarely, cancer can develop in a long-standing fistula
Complications of treatment
- Incontinence. Surgery that cuts too much sphincter muscle can cause leakage of gas or stool. This is why the tract type and surgeon's skill matter.
- Recurrence. Some fistulas return after surgery, especially complex ones.
- Delayed healing, bleeding, infection or scarring
- Harm from unregulated treatment. Unlicensed procedures and home preparations can cause infection or damage the muscle.
Higher risk groups
- People with Crohn's disease
- People with diabetes or weak immunity
- People with tuberculosis
- Those with multiple or high tracts
Warning signs
- Severe pain and swelling with fever
- Spreading redness
- Heavy bleeding
- Unable to pass stool or urine
- New leakage of stool
Seek same-day care. Call 112 or 102 in an emergency.
Reducing risk
Early assessment, treating abscesses promptly, choosing a qualified surgeon, treating underlying disease and following up all lower the chance of complications.
