Introductory summary
The main types are alpha and beta thalassemia, each ranging from silent carrier and trait forms to intermedia and severe major disease.
Types of thalassemia
Thalassemia is classified by which globin chain is affected and how many gene copies are faulty.
Alpha thalassemia (two pairs of alpha genes):
- Silent carrier: one faulty alpha gene. No symptoms, normal blood counts; detectable only by special tests.
- Alpha thalassemia trait: two faulty genes. Mild anaemia, no treatment needed.
- Haemoglobin H disease: three faulty genes. Moderate-to-severe anaemia, enlarged spleen, sometimes needing transfusions.
- Alpha thalassemia major (hydrops fetalis): all four genes faulty. Fatal before or shortly after birth without intensive intervention; prenatal diagnosis matters most here.
Beta thalassemia (more common in India):
- Beta thalassemia minor (trait): one faulty gene. Mild anaemia, healthy life - but a carrier who can pass the gene on.
- Beta thalassemia intermedia: two faulty genes with milder effect. Moderate anaemia; transfusions only when needed.
- Beta thalassemia major: two severely faulty genes. Severe anaemia starting in infancy; lifelong regular transfusions plus iron chelation.
Related conditions often tested together: sickle cell disease, HbE disease (common in eastern India) and combinations like sickle-beta-thalassemia. Carrier screening panels usually cover these at the same time.
Why the type matters: carriers of any type are healthy, but couples where both partners carry beta thalassemia - or one carries beta and the other HbE or sickle - face a risk of a severely affected child with every pregnancy. Type and partner status together guide prenatal decisions.
Remember: trait is harmless; two carriers pairing is the risk scenario; a blood test names your exact type.
Ayurvedic view (interpretive)
Ayurvedic classification does not map onto globin genetics. Typing comes from haemoglobin studies (HPLC/electrophoresis) and guides all real decisions.
