Causes of ulcerative colitis

Introductory summary

Ulcerative colitis is a chronic autoimmune inflammation of the colon - not an infection, not caused by diet or stress; genes, immune misfiring and gut environment combine to trigger it.

Causes of ulcerative colitis

Ulcerative colitis (UC) is a chronic inflammatory bowel disease in which the immune system mistakenly attacks the inner lining of the large intestine (colon and rectum), causing continuous inflammation and tiny ulcers. Understanding what it is - and what it is not - removes a lot of unnecessary guilt and wrong treatment.

What actually drives it:

  • Immune misfiring (the core cause): UC is an autoimmune-type disease. The immune system, meant to attack germs, inappropriately targets the colon lining and keeps the inflammation switched on.
  • Genetics: UC runs in families to a degree - having a close relative with UC or Crohn's raises risk, though most patients have no family history.
  • Gut environment: changes in gut bacteria and the mucus barrier are involved in triggering and sustaining the inflammation; research here is active.
  • Environmental factors: UC is more common in urban, industrialised settings. Interestingly, smoking shows a complex association (non-smokers have higher UC risk - this is NOT a reason to smoke, which harms everything else).
  • Triggers of flares (not causes): stopping maintenance medicines, certain painkillers (NSAIDs), gut infections, and stress can all trigger a flare of existing UC.

What does NOT cause ulcerative colitis:

  • Not an infection - you cannot catch UC from food, water or another person
  • Not caused by eating spicy food, milk, or any diet - diet influences symptoms during flares but does not create the disease
  • Not caused by stress, anxiety or "thinking too much" - stress worsens flares; it did not cause the disease
  • Not the patient's fault - an important point, since many patients blame themselves for years

Why the cause matters: because UC is immune-driven, the effective treatments are anti-inflammatory and immune-modulating medicines - not antibiotics, not antacids, and not diet changes alone.

Remember: UC is the immune system attacking the colon lining - genetic and environmental, never infectious, never caused by food or stress. Flares have triggers; the disease has biology.

Ayurvedic view (interpretive)

Ayurveda would discuss a chronic bloody diarrhoea under Raktatisara or Pitta Grahani. These are descriptive parallels; the autoimmune mechanism is a modern finding, and modern medicines are what control it.
