There is no blood test, scan or genetic panel that diagnoses autism - diagnosis is developmental, built from careful observation and history by trained professionals. That is actually good news: it means the path to answers starts with your paediatrician, not with expensive investigations. What matters is getting the right assessment, early.

Who Diagnoses and How

A proper assessment is done by a developmental paediatrician, child psychiatrist or clinical psychologist - ideally a team. It combines:

• Detailed developmental history with parents: pregnancy, milestones, when words came, play, social responses, sensory reactions
• Structured observation of the child: standardised tools such as ADOS (a play-and-interaction based observation), and interviews like ADI-R or ISAA (the Indian Scale for Assessment of Autism, widely used in India)
• Hearing test - always: hearing loss can mimic autism's speech and response pattern and must be excluded
• Speech-language, cognitive and adaptive assessment: to map strengths and needs, not just confirm a label
• Medical review for co-occurring conditions: sleep, feeding, epilepsy signs, ADHD, anxiety

What Tests May Be Added (and Why)

• Genetic testing (chromosomal microarray, Fragile X): increasingly recommended - it sometimes finds a cause, informs recurrence risk for the family, and occasionally guides care
• EEG if episodes suggest seizures; metabolic tests in specific clinical pictures
• Lead or other tests only when the history points that way
• Not routine: MRI brain scans, "heavy metal" panels, allergy panels - these do not diagnose autism and are often sold as if they do

Your paediatrician or the district early-intervention centre; government support includes assessment and certification under the RPwD Act 2016 (autism is a recognised disability, enabling benefits), and schemes like ADIP for assistive devices. An autism diagnosis also opens UDID card benefits - ask the assessing team.

If You Are an Adult Suspecting Autism

Adult assessment exists and can be life-changing - through a psychiatrist or clinical psychologist experienced in adult autism. Bring school reports and someone who knew you as a child if possible; self-recognition is valid, and a formal opinion opens accommodations at work and study.

Does a diagnosis "label" my child harmfully? Diagnosis done well is a key, not a brand: it unlocks therapies, school accommodations and government benefits, and it gives the child (eventually) a truer story about themselves than "lazy," "stubborn" or "odd."
