Burns Complications and Risks

Introductory summary
Burn complications: infection, contractures, scars, emotional recovery - and how each is prevented.
Most small burns heal cleanly. But burns can go wrong in ways worth knowing in advance - some immediate, some slow.
Early complications
- Infection: the burn wound's main enemy - increasing pain, pus, spreading redness, fever, foul smell. Burns lose skin's protection; infection turns a 2-week heal into a crisis. Clean dressings and early review when signs appear
- Dehydration and shock: large burns leak fluid - this is why big burns are hospitalized urgently
- Hypothermia: a risk DURING first aid - cool the burn, not the whole person; keep the rest of the body warm, especially children
- Airway injury: smoke and hot-air inhalation - hoarseness, soot, breathlessness after a fire is an emergency even with small skin burns
- Sepsis: untreated wound infection spreading to the blood - the reason deep or large burns are never watched at home
Late complications
- Hypertrophic scars and keloids: raised, itchy, thick scars - more common in deeper burns, darker skin, and over the chest and shoulders; silicone sheets, pressure garments and early scar care reduce them
- Contractures: healing skin shortens and pulls - over fingers, elbow, armpit, neck it can permanently limit movement. Movement exercises, splints and physiotherapy during healing prevent it; established contractures may need surgery
- Pigment changes: new burn skin pigments unevenly; strict sun protection for a year reduces it
- Chronic itching: very common in healing burns; moisturizers, antihistamines, patience
- Psychological: burns leave emotional marks - visible scars, accident memories, body-image struggles; counseling and support groups are real parts of recovery
Special-situation complications
- Children: scarring affects growing bodies differently; growth can worsen contractures - pediatric follow-up matters
- Diabetics and the immunosuppressed: slower healing, higher infection risk - lower thresholds for every decision
- Circumferential burns (circling a limb or chest): swelling can strangle blood flow or breathing - emergency territory
The classical note
Classical wound texts warned that a wound wrongly treated early becomes the difficult wound later - and prescribed staged care to prevent exactly that: clean early, protect through healing, correct the scar at the end. Burn complications are mostly preventable in stages 1 and 2; they are only expensive in stage 3.
One-line summary
Fear infection in week one, contracture in month one, and thick scars in year one - nearly all of it is preventable with clean early care, movement, and sun protection; the rest needs a burn specialist early, not late.
