How to Treat Burns

Introductory summary
Burn treatment step by step: the 20-minute cooling rule, home vs hospital criteria, recovery and scar care.
Burn treatment is a ladder: first aid at the bottom (everyone should know it), home care for minor burns, hospital care for everything else. Here is the whole ladder.
Rung 1: First aid (do this immediately, every time)
- Stop the burning: remove the source; smother flames (stop-drop-roll, thick blanket); for chemicals, brush off dry powder then irrigate
- Cool running water for 20 minutes - the single most valuable burn action ever studied
- Remove jewelry and constricting clothing before swelling
- Cover loosely with clean cling film, a clean bag, or a clean damp cloth
- Keep warm, give nothing to eat or drink if the burn looks serious
- Decide: home or hospital (criteria below) - when in doubt, hospital
Rung 2: Home care (minor burns only)
Small (palm-sized or less), superficial (red or small blisters), away from face/hands/genitals/joints, in a healthy adult or older child: continue as in the home-remedies piece - clean, covered, pain relief, watch for infection, moisturize and sun-protect the new skin.
Rung 3: Doctor or hospital (everything else)
Go in for: anything larger than the palm, any white/charred/numb area, burns on face, eyes, ears, hands, feet, genitals, or crossing a joint, all chemical and electrical burns, breathing symptoms or soot around the nose, burns circling a limb, and burns in babies, the elderly, pregnant women, diabetics or the immunosuppressed. Hospitals offer specialized dressings, tetanus cover, infection surveillance, and - for deep burns - skin grafting. Emergencies (large, deep, airway, electrical, unconscious): call 112 / 102.
Rung 4: Recovery and scar care
New skin: moisturize, sun-protect for a year. Scars: silicone sheets, pressure garments, physiotherapy for anything crossing a joint, and a plastic-surgery opinion for thick or tightening scars. Emotional recovery counts: support is part of burn care.
The classical ladder
The classical sequence - cool and clean (shodhana), heal (ropana), restore (vaikrutapaha) - maps onto the same rungs, and its healing-phase formulations (Shatadhauta ghrita, Jatyadi taila) sit on rung 2 for minor burns. One honest divergence, stated plainly: where classical texts suggest warm applications on a fresh scorch, modern burn science says cool running water 20 minutes - follow the water. The first half-hour belongs to modern first aid; tradition joins in the healing weeks.
One-line summary
Cool, cover, decide; home care for the small and superficial; hospital for the rest; moisturize, protect and support through recovery - and let water, not any product or paste, be the first thing that touches every burn.
