Medicines and Products for Arthritis
Arthritis medicines range from simple pain relief to sophisticated immune-targeting drugs - and the right one depends entirely on the type. The framework that matters: some medicines treat symptoms, some change the disease itself, and the most dangerous ones are the unregulated products sold as cures, per NHS guidance on arthritis treatment.
Osteoarthritis: Symptom Control
- Paracetamol: modest benefit; safe at correct doses - do not exceed limits or stack combination products containing it
- NSAIDs (ibuprofen, diclofenac and cousins): effective for OA pain and flares, best used at the lowest dose for the shortest stretch; they carry stomach, kidney, blood-pressure and heart risks with sustained use - topical NSAID gels deliver much of the benefit with far less risk and are underused
- Capsaicin cream: genuine option for hand and knee OA
- Steroid injections into the joint: useful for flares, a few times per joint per year at most
- What does not work: glucosamine and chondroitin for most people; collagen boosters; "cartilage repair" supplements - save the money
Rheumatoid Arthritis: Disease-Modifying Treatment
- DMARDs (methotrexate first, plus sulfasalazine, hydroxychloroquine, leflunomide): the backbone - they suppress the immune attack and prevent joint destruction. Methotrexate is safe under monitoring (regular blood tests), and starting it early is the single decision that most changes the disease's whole future
- Biologics and targeted drugs (TNF blockers and others): for disease that outruns standard DMARDs - highly effective, with infection precautions
- Steroids: powerful short-term bridges during flares or while DMARDs take effect - and a trap as long-term self-medication: weight gain, bone thinning, diabetes, infection risk. Steroid courses belong to doctors, never to repeat at will
- Pain relief alongside: NSAIDs help symptoms but do not slow damage - pain control is not disease control
Gout: Treat the Attack and the Cause
- For attacks: NSAIDs, colchicine, or steroids - started early, they shorten flares dramatically
- For the cause: allopurinol (and alternatives) lower uric acid and prevent both attacks and joint/kidney damage. Taken daily, long-term, with a target level - not just during attacks. Starting it can briefly provoke flares, which is managed, not feared
- The commonest gout mistake: treating attacks forever while never lowering the level
Products to Be Wary Of
- "Arthritis cure" mixtures and pain balms with undisclosed steroids: a real and documented hazard - miraculous instant relief from an unknown powder often means hidden steroid, with all its harms unmonitored
- Online "herbal DMARDs" or stem-cell promises: unproven, expensive, dangerous as substitutes
- Copper bracelets, magnets, miracle devices: no evidence
Frequently Asked Questions
Is methotrexate dangerous? It is a serious medicine used safely by millions under monitoring - blood tests every few weeks at first, then periodically. The risks of untreated RA (destroyed joints, heart and lung disease) far outweigh its managed risks.
Are painkillers enough for rheumatoid arthritis? Never - they quiet symptoms while the disease keeps eroding joints. Only DMARDs change the disease. Pain relief rides along; it cannot lead.
Can I stop allopurinol once gout attacks stop? Stopping lets uric acid rise and attacks return - usually within months. Gout treatment is long-term by design; discuss targets and duration with your doctor rather than stopping unilaterally.
Are steroid tablets safe to keep at home for flares? Only as part of a plan agreed with your doctor, with clear dose and duration limits. Self-repeating steroid courses is one of the commonest and most harmful patterns in arthritis care.
