Introductory summary
Osteoarthritis, rheumatoid arthritis, gout and the other major types: how they differ, and which ones cannot wait.
Types of Arthritis
Over a hundred conditions cause joint inflammation, but a handful account for nearly everyone you meet with arthritis. Each type has its own cause, pattern, treatment and stakes - and telling them apart is the first job of any joint assessment.
Osteoarthritis (OA)
The commonest type by far - the "wear and repair" arthritis. Cartilage thins and the bone beneath responds, producing pain with use, brief stiffness, and bony enlargement, typically in knees, hips, hands and spine. It progresses slowly and stays in the joints - it never causes fever or whole-body illness. Management centres on strengthening, weight, pain control, and - when end-stage - joint replacement, which is among the most successful operations in medicine.
Rheumatoid Arthritis (RA)
The most important type to catch early: an autoimmune attack on the joint lining, causing symmetric hot, swollen small joints, prolonged morning stiffness and systemic illness. Untreated, it erodes joints within years and can affect lungs, heart, eyes and blood vessels. It is also profoundly treatable: modern DMARD therapy (methotrexate and its successors), started early, lets most people live without deformity. RA is the arthritis where months matter - early rheumatology review changes the whole disease.
Gout
Crystal arthritis from excess uric acid: sudden savage attacks, classically the big toe, then other joints; between attacks the joint feels normal until chronic gout laces joints with crystal deposits (tophi). Very treatable - attack relief plus long-term uric-acid-lowering medicine - and very mistreated when people take painkillers for attacks but never treat the underlying level.
Other Types Worth Knowing
- Psoriatic arthritis: inflammatory arthritis accompanying psoriasis - can damage joints as aggressively as RA and needs the same early DMARD approach
- Ankylosing spondylitis (axial SpA): inflammatory back arthritis starting in young adults - chronic morning back stiffness that improves with exercise, not rest
- Juvenile idiopathic arthritis (JIA): childhood autoimmune arthritis - a limping child or a persistently swollen joint is never "just growing pains"
- Septic arthritis: bacterial infection inside a joint - the emergency type; a hot red joint with fever needs same-day care
- Reactive arthritis: joint inflammation following certain gut or urinary infections
The Ayurvedic Perspective
Classical medicine recognised distinct joint diseases with a similar instinct for pattern: Sandhivata (dry, degenerative, use-worn joints - the OA pattern), Amavata (systemic illness with Ama settling in symmetric small joints - the RA pattern), and Vatarakta (burning, crystal-like affliction of blood and joints, classically the great toe - the gout pattern) (Charaka Samhita, Chikitsa Sthana 28-29; Madhava Nidana, Chapters 22-25). The correspondences are interpretive, not diagnostic - but they show how long these patterns have been observed.
Frequently Asked Questions
Which type is most serious? Different dangers: septic arthritis kills joints in days (emergency), untreated RA deforms in years (urgent treatment), gout damages joints and kidneys when its cause is ignored, and OA grinds down quality of life over decades. All are treatable - the mistake is delay.
Can someone have two types at once? Yes - gout can land on an osteoarthritic joint, and RA patients can develop OA in damaged joints. Each type keeps its own treatment.
Is "rheumatism" a type? No - it is a loose old word for joint and muscle aches. A specific diagnosis (OA, RA, gout...) is what unlocks the right treatment.
Does psoriasis always lead to psoriatic arthritis? No - a minority of people with psoriasis develop it. New joint pain or swelling in someone with psoriasis deserves rheumatology review, whatever the skin is doing.
