Stages of Diabetes Mellitus

Reviewed by medical experts · For informational purposes only

Introductory summary

Silent insulin resistance, reversible prediabetes, established diabetes, and the complications only neglect brings - every stage has a window.

Diabetes is not a single event but a progression - and each stage has its own window where the right action changes the ending.

Stage 1: Insulin resistance (silent)

  • The body's cells stop responding well to insulin; the pancreas compensates by making MORE
  • Blood sugar is still normal - no test of sugar finds this stage; the signs are the waist growing, the family history, the sedentary years
  • Window: widest of all - lifestyle change here prevents everything downstream

Stage 2: Prediabetes (the warning)

  • The pancreas starts losing the compensation battle: fasting sugar 100-125 mg/dL or HbA1c 5.7-6.4%
  • Still usually no symptoms - found only by testing
  • Window: genuinely reversible - structured diet, 150 minutes of weekly activity and 5-7% weight loss cut progression by more than half in the big studies

Stage 3: Established diabetes

  • Sugar crosses the diagnostic line: fasting 126+, HbA1c 6.5%+
  • Symptoms may appear - thirst, frequent urination, fatigue - or nothing at all
  • Window: control is the word now - good sugar control prevents or delays every complication

Stage 4: Diabetes with complications

  • Years of high sugar damage the small vessels: eyes (retinopathy - a leading cause of adult blindness), kidneys (nephropathy - toward dialysis), nerves (neuropathy - numb feet, then injuries that do not heal)
  • And the large vessels: heart attack and stroke risk roughly double
  • The diabetic foot: numbness plus poor circulation plus infection - the pathway to ulcers and amputation, and the reason daily foot checks are prescribed

One-line summary

Diabetes progresses from silent insulin resistance, through reversible prediabetes, into established diabetes and - only if neglected - the complications of eyes, kidneys, nerves, heart and feet; every stage has a window, and the earliest ones close quietly.
