Symptoms of Diarrhea

Reviewed by medical experts · For informational purposes only

Introductory summary

Watch the water, not just the stool: thirst, dry mouth, scanty urine, sunken eyes and lethargy mark the dehydration that makes diarrhea dangerous.

Diarrhea itself is a symptom - the pattern around it tells you how serious it is and what to do. The dangerous symptoms are not in the stool; they are in the body losing water.

The basic picture

  • Loose, watery stools three or more times a day
  • Cramping abdominal pain, urgency, sometimes bloating and nausea
  • Fever when the cause is infectious

The dehydration signs (the ones that matter)

  • Thirst, dry mouth and tongue
  • Very little urine, dark urine - in children, a dry diaper for 4-6 hours
  • Sunken eyes, lethargy or unusual irritability
  • Skin pinch on the abdomen that returns slowly
  • Dizziness on standing; in infants, a sunken fontanelle (the soft spot)
  • No tears when crying, in a child

The danger signs - doctor today

  • Blood or mucus in the stool, or black tarry stool
  • High fever with severe abdominal pain
  • Signs of moderate-severe dehydration: sunken eyes, lethargy, very little urine, rapid breathing
  • Diarrhea lasting more than 2-3 days despite good fluid intake
  • In infants under 6 months, the elderly, pregnant women, and anyone with a weak immune system: a lower threshold - earlier is safer
  • Persistent vomiting that keeps even ORS down impossible

The chronic pattern

  • Diarrhea lasting over 2-4 weeks, weight loss, night symptoms, anemia: this is no longer "a stomach bug" - it needs a workup for celiac, IBD, parasites and more

One-line summary

Watch the water, not just the stool: thirst, dry mouth, scanty urine, sunken eyes and lethargy mark the dehydration that makes diarrhea dangerous - and blood, high fever or an infant or elder with these signs means the doctor today.
