Complications and risks of scabies

Reviewed by medical experts · For informational purposes only

Introductory summary

Complications of scabies and when to seek urgent care.

Scabies is usually a short-term problem that clears fully with treatment. Complications are more likely when it goes untreated for a long time, when the skin is broken by scratching or when the immune system is weak.

Skin infection Scratching opens the skin and lets bacteria such as Staphylococcus aureus and Streptococcus in. This can cause impetigo (honey-coloured crusts), boils, abscesses and cellulitis, with pain, swelling, warmth and pus.

Deeper and general infection In rare cases, bacterial infection can spread to the blood (sepsis). Certain streptococcal infections can lead to kidney inflammation (post-streptococcal glomerulonephritis) and, rarely, rheumatic fever, particularly in children in crowded settings.

Long-lasting itch and nodules Itchy lumps (nodular scabies) and post-scabies itch can last weeks to months, even after the mites are cleared.

Sleep and mental health effects Constant night itching disturbs sleep and affects school, work and mood. Stigma can cause shame or anxiety, even though scabies is not due to poor hygiene.

Crusted scabies In people with weak immunity, the elderly and those with disabilities, crusted scabies carries a high mite load and causes outbreaks in care homes and hospitals. It may need isolation and prolonged treatment.

Skin changes Darker or lighter marks and scars may remain after healing, particularly if the skin was scratched or infected.

When to see a doctor urgently Seek care promptly for fever, spreading redness, pus, severe pain, swollen glands, dark urine or swelling of the face and legs. Babies with scabies and anyone with thick crusts need prompt review.

Ayurvedic view (interpretive) Texts note that neglected skin disease can become deep-seated. This is a conceptual parallel, not a replacement for medical care.

Remember Early treatment and not scratching prevent most complications.
