Complications and risks of sciatica

Reviewed by medical experts · For informational purposes only

Introductory summary

Complications of sciatica, including cauda equina warning signs.

Most people with sciatica recover well, and serious complications are uncommon. However, if the nerve stays compressed or the cause is not treated, lasting problems can develop.

Chronic pain Pain that continues for more than three months can become long-term. It can affect sleep, mood, work and daily activities, and may lead to anxiety or low mood.

Nerve damage

  • Numbness or reduced feeling in the leg or foot.
  • Muscle weakness and wasting.
  • Foot drop, where it becomes hard to lift the front of the foot, causing tripping.
  • Permanent damage is possible if severe pressure on the nerve continues without treatment.

Cauda equina syndrome (emergency) A rare but serious complication in which the nerves at the base of the spine are compressed. Warning signs include numbness in the groin or inner thighs (saddle area), difficulty passing or controlling urine or stool, sexual dysfunction and weakness in both legs. It needs emergency surgery within hours to protect function. Call 112 or go to the nearest hospital.

Reduced activity Fear of movement can lead to weak muscles, weight gain and stiffness, which can make pain worse.

Recurrence Sciatica can return, particularly if the cause, such as a weak core or poor posture, is not addressed.

Side effects of treatment Long use of anti-inflammatory tablets can affect the stomach and kidneys, and nerve pain medicines can cause drowsiness. Surgery has risks such as infection, bleeding and, rarely, nerve injury.

Ayurvedic view (interpretive) Texts describe untreated Vata disorders as leading to wasting and loss of function. This is a conceptual parallel and does not replace medical care.

Remember Seek early help for weakness, numbness or bladder changes. Early treatment protects the nerve.
