Ayurvedic treatment for sciatica

Introductory summary
Ayurvedic view of gridhrasi as an adjunct, with safety notes.
Ayurveda describes sciatica as gridhrasi, named after the vulture-like walk of a person in pain. In the traditional view, aggravated Vata (and sometimes Kapha) lodges in the lower back and leg along the course of the nerve. This is an interpretive framework. Evidence for Ayurvedic treatments of sciatica is limited and mixed, so they should be an adjunct to modern assessment and not a substitute for it.
Traditional approach
- Oil massage (abhyanga) of the back and leg with warm medicated oils such as sesame-based preparations, followed by gentle warming (swedana).
- Basti (medicated enema) is the classic treatment for Vata disorders, and is practised only under a qualified practitioner.
- Internal herbs such as ashwagandha, guggulu, rasna, shunthi (dry ginger), nirgundi and castor oil are often prescribed.
- Warm, light, easily digested food, and avoiding cold, dry and gas-forming food.
- Gentle daily movement and avoiding long sitting and cold exposure.
What research says A few small trials suggest that some Ayurvedic therapies may reduce pain, but they are limited and vary in quality. Good evidence exists for staying active, physiotherapy and, in the right cases, medicine.
Safety points
- Do not accept forceful spinal manipulation or heavy massage over a painful disc without a doctor's assessment.
- Some herbal and mineral products contain heavy metals, and guggulu may interact with medicines. Tell your doctor about all products.
- Basti and strong therapies are not advised in pregnancy or frailty without specialist advice.
- Never delay care if there are red flags.
When to see a doctor urgently Leg weakness, numbness in the groin, loss of bladder or bowel control, or severe pain after injury need urgent medical care. Call 112.
Remember Ayurvedic oil massage and warmth may bring comfort, but ensure that serious causes are ruled out first.
