Introductory summary
Viral tonsillitis is treated with rest, fluids and pain relief for a week; strep gets a full antibiotic course; recurrent tonsillitis may end with tonsillectomy on criteria.
How to treat or cure tonsillitis
Step 1: Identify the likely type. Cold symptoms with sore throat = viral, treated at home. High fever, severe pain, pus on tonsils, tender neck glands and no cough = likely strep, needing a doctor's assessment (throat swab or clinical scoring). Drooling, breathing difficulty, muffled voice or one-sided swelling = emergency department now.
Step 2: Viral - supportive care. Rest, fluids, salt-water gargles, honey (over 1 year), soft foods, paracetamol or ibuprofen. It resolves in 3-7 days. Antibiotics do not help and do harm when misused.
Step 3: Strep - antibiotics, completed. Penicillin/amoxicillin (or an alternative for allergy) for the full prescribed course. Improvement comes in 24-48 hours, but finishing the course is what prevents rheumatic fever - the complication that damages heart valves years later. Keep the child home until fever-free and 12-24 hours into treatment.
Step 4: Complicated and recurrent courses.
- Peritonsillar abscess: drainage plus IV antibiotics in hospital.
- Mononucleosis: rest, fluids, no contact sports while the spleen is enlarged.
- Recurrent tonsillitis: an ENT specialist documents attack frequency; tonsillectomy is recommended when episodes meet standard criteria (about 7 in one year, 5/year for 2 years, or 3/year for 3 years), or when tonsils obstruct sleep (snoring, apnea pauses). Surgery ends tonsillitis for good in those selected patients.
Step 5: Follow-up. Recheck if symptoms last beyond a week, worsen after improving, or if joint pains, rash or dark urine appear weeks after a sore throat (post-strep complications).
Remember: comfort for viral, completed antibiotics for strep, surgery only for the properly counted recurrent cases.
