Symptoms of urinary tract infection

Introductory summary
A bladder UTI burns, stings and makes you urinate small amounts urgently and often; fever with flank pain means the kidneys are involved - that needs a doctor the same day, not home remedies.
Symptoms of urinary tract infection
Lower UTI (bladder/urethra - the common form):
- Burning or stinging while passing urine - the classic complaint
- Frequency - needing to urinate every few minutes, day and night
- Urgency - a sudden, hard-to-hold need to go
- Passing only small amounts each time, with a feeling of incomplete emptying
- Cloudy, strong-smelling or blood-tinged urine
- Lower abdominal heaviness or pain above the pubic bone
Upper UTI (kidney infection - pyelonephritis) - the serious form:
- High fever, often with chills and rigors
- Pain in the flank - the side/back below the ribs, often one-sided
- Nausea and vomiting
- The bladder symptoms above may or may not accompany it
- This is not a home-treatment situation - kidney infections can spread to the bloodstream; see a doctor the same day
Sepsis warning signs - call 112: very high fever with shaking chills, confusion, rapid breathing, extreme weakness, or a drop in blood pressure. A urinary infection becoming bloodstream infection is a medical emergency, especially in the elderly and diabetics.
Special patterns worth knowing:
- In elderly patients, a UTI may cause no burning at all - new confusion, drowsiness or a fall can be the only sign
- In young children, watch for fever without a clear source, poor feeding, foul-smelling urine or new bedwetting
- In pregnancy, UTIs can be completely silent - which is why urine tests are routine at antenatal visits
What UTI symptoms are NOT: vaginal itching with discharge points to a vaginal infection, not a UTI; painless blood in urine without infection signs needs evaluation for other causes including stones and, in older adults, bladder growths.
Remember: burning + frequency + urgency is the bladder pattern - very treatable. Fever + flank pain is the kidney pattern - same-day doctor. Confusion in an elder can be the whole presentation.
Ayurvedic view (interpretive)
Mutrakrichra texts describe burning, frequency and scanty flow accurately at the bladder level. The kidney and sepsis patterns, however, are emergency medicine - no traditional observation replaces antibiotics there.
