Medicines and products for vitiligo

Introductory summary

Steroids and tacrolimus for limited patches, phototherapy for wider disease, grafting for stable patches - early treatment repigments best.

Vitiligo treatment has genuinely effective options - the best results come from starting early and matching the product to the phase and site of disease. A dermatologist should drive the choices.

Topical medicines (first line for limited disease):

  • Topical corticosteroids: the standard first treatment for active, limited patches - they calm the immune attack and can return colour. Potency and duration must be supervised: long unsupervised use thins skin, and steroid-containing 'fairness' creams bought over the counter are a real source of harm.
  • Calcineurin inhibitors (tacrolimus, pimecrolimus): steroid-sparing options especially good for the face and folds, safe for longer use.
  • Newer targeted options: topical ruxolitinib (a JAK inhibitor) is an approved advance for repigmentation in some countries - ask your dermatologist about availability.

Phototherapy. Narrowband UVB, two to three sessions weekly over months, is the mainstay for widespread or spreading disease and the most reliable repigmenting tool. Clinic-based is standard; supervised home units exist.

Oral medicines. Short, supervised mini-pulse steroid courses can halt rapidly spreading disease. Antioxidants are sometimes added as support.

Surgery. For patches stable for at least a year (especially segmental type), grafting techniques (suction blister, punch, melanocyte-cell suspension transplants) transfer pigment cells into white patches, with good cosmetic results on suitable sites.

Depigmentation. When disease is very extensive, removing the remaining pigment (monobenzone) for an even tone is an honest option some patients choose.

Products to avoid: over-the-counter 'vitiligo cure' creams (often undeclared steroids), unverified herbal applications, and any product promising guaranteed or rapid cure.

Ayurvedic view (interpretive): bakuchi-based products are the classical offering within an interpretive framework of limited evidence and carry photosensitization risk - adjunct at most, supervised, never replacing the options above.

Remember: steroids and tacrolimus for limited patches, phototherapy for wider disease, surgery for stable patches - early treatment repigments best. Guaranteed-cure products are the red flag.
