Stages of Cataract

Introductory summary
From incipient haze to mature and hypermature cataract - staging decides timing; the classics staged it two millennia ago.
Cataract progresses from "glasses fix it" to "surgery fixes it" to "waiting made surgery harder." Knowing the stages tells you exactly where you stand.
Stage 1: Early (immature)
Mild clouding; symptoms limited to slight blur, some glare, a glasses-power change. Management: updated glasses, brighter lighting, sunglasses outdoors, and a review schedule. No surgery yet - but the annual eye check starts here, and diabetes control (if diabetic) genuinely slows the march.
Stage 2: Progressing
Vision now interferes: reading needs magnification, night driving is out, faces blur at a distance, work suffers. This is the surgery-decision zone - not based on a scan number but on life: when vision limits what you do, the lens comes out. Waiting for the cataract to "ripen" fully is outdated advice; modern surgery does not need maturity.
Stage 3: Mature
The whole lens is opaque; vision is hand-movements or light-perception level. Surgery still restores sight - but the eye has spent years unexercised, and falls, dependence and accidents have accumulated. Restoring sight now is still one of medicine's most dramatic wins.
Stage 4: Hypermature (the penalty stage)
Left longer, the lens leaks protein (inflaming the eye - phacomorphic/phacolytic glaucoma) or swells and pushes pressure up. Now there is pain and urgency, surgery is technically harder, and some damage (from the pressure) can be permanent. Every hypermature cataract is a stage-2 decision postponed.
The children's clock is different
Congenital and infantile cataracts run on the brain's development schedule, not the lens's: operate too late and the brain never learns to see with that eye (amblyopia). Weeks-to-months matter, not years.
The classical staging - two millennia early
Sushruta staged this disease as timira (early misting, medicine-stage), kacha (advancing), linganasha (mature - surgical stage) - and his text graded which mature cases were operable and which were not. The classical eye surgeons drew the same line modern ones draw: medicines do not fix a mature cataract; surgery does.
The classical staging, in detail
The classics mapped the same progression as doshas penetrating four layers (patalas) of the pupil region (Sushruta Uttara ch. 7; Madhava Nidana 59). First patala: hazy vision, as if through rain or smoke (early timira). Second: false artifacts - flies, cobwebs, halos, doubling (advanced timira). Third: only large objects visible, and the pupil changes color by dosha - greyish-copper in vataja, blue-green in pittaja, glistening pearl-white in kaphaja (kacha). Fourth: total opacity, light perception alone (linganasha). The prognostic grading followed: first and second patala disease is medical territory (sadhya); fully mature kaphaja linganasha - firm, smooth, pearl-white - is the surgical case (shastra-sadhya); and the vataja, pittaja, raktaja and sannipataja mature types were declared asadhya - do not operate. A grading of surgical fitness written two millennia before operating microscopes.
One-line summary
Glasses stage, life-limiting stage (operate), mature stage (still operable), hypermature stage (harder, partly irreversible) - the right stage to act is when vision starts costing you your life.
