Causes of Celiac Disease

Reviewed by medical experts · For informational purposes only

Introductory summary

Genes (HLA-DQ2/DQ8), gluten and an immune switch cause celiac - removing gluten stops the whole process.

Celiac disease needs three things together: the genes, the gluten, and a trigger that switches the immune system on. Remove gluten and the whole process stops - that is why the diet works.

The genes

Almost everyone with celiac disease carries HLA-DQ2 or HLA-DQ8 gene variants. These genes are common (a third of us carry one) but necessary - without them, celiac disease is essentially impossible. Having the genes is not a sentence; most carriers never develop the disease.

The gluten

Gluten is the storage protein of wheat, barley and rye. In celiac disease, incompletely digested gluten fragments slip through the gut lining and are presented to the immune system in a way that provokes attack. The target of that attack ends up being the intestine's own tissue - autoimmunity, not allergy.

The switch

Why does it start when it starts? Triggers proposed include infections, pregnancy, severe stress, surgery, or changes in gut bacteria - often no clear trigger is found. What matters practically: the disease can appear in childhood or at any adult age, even after decades of eating wheat without trouble.

Who carries higher risk

  • First-degree relatives of a celiac patient (about 1 in 10 chance)
  • Type 1 diabetes, autoimmune thyroid disease, Down and Turner syndromes
  • People with unexplained iron-deficiency anemia or early osteoporosis

These groups should be tested even with mild symptoms.

What does NOT cause celiac disease

Wheat itself does not cause it - billions eat wheat safely. Modern wheat, pesticides or "impure atta" are not proven causes. Overthinking the cause changes nothing; removing gluten changes everything.

One-line summary

Celiac disease happens when the right genes (HLA-DQ2/DQ8), gluten exposure and an immune switch come together - and because gluten is the fuel, removing it is the complete treatment.
