Stages of Celiac Disease

Introductory summary
Marsh grades 0-3 measure intestinal damage; symptoms heal in weeks, the gut in up to two years - mostly reversible.
Celiac disease is not staged like cancer. Instead, doctors grade the intestinal damage on biopsy and track recovery over time. Both matter.
The Marsh grading (what the biopsy report means)
- Marsh 0: normal intestine (usually with positive blood markers - the latent form)
- Marsh 1: increased immune cells in the lining, villi still intact - the earliest visible change
- Marsh 2: the crypts (glands between villi) enlarge; villi still holding
- Marsh 3: villous atrophy - partial (3a), subtotal (3b) or total (3c). This is established celiac disease; absorption is genuinely impaired here
A "Marsh 3c" report sounds frightening but heals on the diet like any other; the grade describes damage, not doom.
The practical timeline
- Before diagnosis: months to decades of accumulating damage and deficiencies
- First weeks gluten-free: symptoms usually improve within days to weeks
- Gut healing: blood markers normalize over 6-12 months; the intestinal lining takes 6 months to 2 years (longer in adults)
- Lifelong maintenance: the disease sleeps only while the diet holds; every regular gluten exposure re-awakens it
What untreated progression looks like
Uncontrolled celiac disease compounds: worsening anemia, bone thinning toward osteoporosis and fractures, persistent infertility, neurological symptoms, and a small but real increase in intestinal lymphoma risk. The rare end-stage is refractory celiac disease - the gut stops responding even to the diet and needs specialist immune treatment.
The healing side of the ledger
Every stage of damage except refractory disease is reversible territory. Villi regrow. Children catch up on growth. Bones re-densify. The staging exists to measure the climb back, not to label decline.
The classical chronicity grading
Madhava Nidana (ch. 4) carried the same instinct: it graded chronic Grahani by who bears it and how far it has gone. Recent disease in children was declared curable (sadhya) - resilient tissue rebuilds fast, which matches the modern experience of children healing fully on the diet. Disease in young adults was krichhrasadhya - curable with long, strict discipline. Long-standing disease in the elderly or emaciated was yapya - palliable, managed rather than reversed. A severe chronic form (ghatiyantra, named for intestines gurgling like a submerged water pot) marked the deep end. The ancients already knew: catch it young, hold the discipline, and the gut comes back.
One-line summary
Biopsy Marsh grades 0-3 measure intestinal damage, and healing runs weeks for symptoms to two years for the gut lining - nearly all of it reversible once gluten truly stops.
