Introductory summary

Antifungal creams and tablets explained, and why steroid combination creams are risky.

Most fungal infections need an antifungal medicine. The choice depends on the site and severity, so see a doctor for diagnosis, especially for scalp, nail or recurrent infections.

Skin infections (creams, lotions, powders)

  • Clotrimazole, miconazole, ketoconazole, terbinafine, luliconazole or sertaconazole are common topical antifungals.
  • Apply as directed, usually once or twice daily, and continue for a couple of weeks after the rash clears, as advised.
  • Use enough to cover the rash and a margin of healthy skin around it.

Oral antifungal tablets

Used for scalp, nail, widespread or stubborn infections, and in some mouth and vaginal infections:

  • Terbinafine, itraconazole, fluconazole or griseofulvin
  • Need a prescription. They can affect the liver and interact with other medicines, so tell your doctor about other medicines and liver problems.
  • Not suitable for everyone, including in pregnancy for some drugs.

Mouth and genital candidiasis

Antifungal gels, lozenges, pessaries or tablets as prescribed.

Steroid and combination creams: a warning

Creams that combine a steroid with an antifungal, such as clotrimazole with betamethasone, are widely sold without a prescription in India. They can relieve itching for a short time, but often make the infection worse, spread it, change how it looks and promote resistance. Do not use them without a doctor's advice.

Other products

Antifungal shampoos, such as ketoconazole, help with scalp and pityriasis versicolor, along with the main treatment.

Safety

  • Do not use leftover medicines.
  • Finish the prescribed course.
  • Tell your doctor if you are pregnant or breastfeeding.
