Symptoms of Chickenpox

Reviewed by medical experts · For informational purposes only

Introductory summary

Fever, then crops of itchy dew-drop blisters with all stages at once - and the warning signs that mean a doctor now.

Chickenpox announces itself in a pattern doctors recognize at a glance: a day or two of feverish unwellness, then the famous itchy blisters arriving in crops. Knowing the pattern separates it from lookalikes.

The opening act

  • Fever (usually mild-moderate in children, higher in adults), tiredness, headache, poor appetite
  • Sometimes a day of feeling "viral" before anything shows on the skin

The rash (the signature)

  • Starts as red spots, turns within hours into fluid-filled blisters on a red base - "dew drops on a rose petal"
  • Appears in CROPS over 3-5 days: new spots keep coming while old ones crust, so all stages (spot, blister, crust) coexist - the classic diagnostic clue
  • Distribution: begins on the scalp, face and trunk, then spreads outward; palms and soles usually spared
  • Itch: often intense, especially at night
  • Crusting: blisters dry into scabs over 5-7 days; scabs fall off over 1-2 weeks
  • Mouth, eyes and genitals can develop painful spots too

Children vs adults

Healthy children: itchy, feverish, miserable for a week, then fine. Adults and teens: higher fever, heavier rash, more body ache - and a real risk of lung involvement.

Warning signs (seek care promptly)

  • High or persistent fever beyond 4 days, or fever returning after settling
  • A blister area turning red, hot, swollen, painful or pus-filled (bacterial infection)
  • Breathlessness, persistent cough, chest pain (lung involvement)
  • Confusion, stiff neck, unsteady walking, severe vomiting (brain involvement - rare)
  • Spots near or in the eye
  • Any case in pregnancy, a newborn, or someone with weak immunity - do not wait

One-line summary

Fever, then crops of itchy "dew drop" blisters with all stages present at once, crusting over a week - and warning signs (breathlessness, eye spots, spreading redness, pregnancy) that mean a doctor now.
