Types of Chikungunya

Reviewed by medical experts · For informational purposes only

Introductory summary

Acute, post-acute and chronic - chikungunya's phases, and the chronic joint phase that needs a specialist's long game.

Doctors classify chikungunya less by "types" of virus and more by the phase the illness is in - because the phases behave differently and are managed differently.

Acute chikungunya (days 1-14)

The viral storm: high fever, severe joint pain, rash, exhaustion. The virus is active in the blood; care is supportive - rest, fluids, paracetamol. Most people pass through and recover here.

Post-acute chikungunya (weeks 2-12)

Fever is gone, but joints still ache, stiffen and swell - wrists, ankles, fingers especially. Fatigue lingers. This phase tests patience: it is inflammation unwinding, not ongoing infection.

Chronic chikungunya (beyond 3 months)

A substantial minority - estimates range from a fifth to nearly half of adults - carry joint pain, stiffness and swelling past three months, sometimes for years, flaring and settling. It resembles rheumatoid arthritis in pattern. Older age and severe acute joint pain raise the odds. This phase needs proper rheumatology-style care: anti-inflammatory treatment, graded exercise, sometimes stronger immune-modulating drugs under a specialist.

Atypical and severe presentations

  • Newborn infection (mother ill around delivery): fever, poor feeding, rash - hospital care
  • Neurological: rare - confusion, seizures, nerve inflammation
  • Eye, heart and kidney involvement: uncommon, serious
  • These belong to hospitals, not home care

By blood test, not by feel

Acute, post-acute and chronic describe timing; the diagnosis itself is confirmed by tests (RT-PCR early, antibody IgM later). Joint pain after "a viral fever" months ago deserves the chikungunya antibody test - many chronic cases are diagnosed in hindsight.

One-line summary

Acute (two weeks), post-acute (to three months) and chronic (beyond) - chikungunya's types are its phases, and the chronic joint phase is the one that needs a specialist's long game.
