Stages of Chikungunya

Reviewed by medical experts · For informational purposes only

Introductory summary

A silent incubation week, a 10-day fever storm, a fork at week 2-4, and a months-long joint tail for the unlucky.

Chikungunya moves through recognizable stages from mosquito bite to full recovery - with one stage (the long joint tail) that surprises patients who expected a one-week fever.

Stage 1: Incubation (2-12 days, typically 3-7)

The mosquito has bitten; the virus is multiplying silently. No symptoms. During an outbreak, anyone with Aedes exposure is a candidate - the fever arrives without warning.

Stage 2: Acute febrile stage (days 1-10)

The storm: abrupt high fever, severe joint pain in wrists, ankles and fingers, headache, exhaustion, and often a rash from day 2-5. This is the contagious-to-mosquitoes window: a patient bitten now can seed new cases. Rest, fluids, paracetamol, mosquito protection FOR the patient (nets, repellent) to break the chain.

Stage 3: Recovery fork (weeks 2-4)

The fever breaks and two paths open:

  • Most: steady improvement - joints settle, energy returns, back to normal within weeks
  • A minority: joint pain and stiffness persist or flare - the fork toward chronic disease

Stage 4: Post-acute / chronic joint stage (weeks to months, sometimes years)

Lingering morning stiffness, joint swelling, tendon pain, and fatigue - worse after rest, easing with gentle movement. Flares come and go. This is inflammatory arthritis after the virus, not ongoing infection: it is managed with anti-inflammatory medicines, graded exercise and sometimes specialist drugs.

The full timeline at a glance

Bite to fever: about a week. Fever storm: about 10 days. Most recoveries: 2-4 weeks. The joint tail in the unlucky: 3 months to 3 years, gradually fading. Knowing this calendar prevents both panic at week 3 and neglect at month 6.

One-line summary

A silent week of incubation, a 10-day fever storm, a fork at week 2-4 - most walk away, some carry joint pain for months - each stage has its own care.
