How to Treat or Cure Cholera

Introductory summary
ORS from the first stool, facility care at dehydration signs, IV fluids and antibiotics for the severe - fluids, escalated fast.
Cholera treatment is a race the fluids almost always win - if started in time. The protocol is one of the best-established in all of medicine, and it scales from kitchen to ICU.
Step 1: Start ORS immediately
At the first watery stools - before any diagnosis, before leaving for the hospital: ORS sips, continuously. Packet ORS per instructions, or the measured homemade sugar-salt liter. This single step decides most outcomes.
Step 2: Judge the dehydration
- No/mild signs: continue ORS at home, watch closely, feed normally
- Moderate (sunken eyes, poor skin pinch, very thirsty, low urine): medical facility the same hour - ORS plus observation; many centers run rapid ORS protocols successfully
- Severe (limp, drowsy, cold hands, no urine, weak pulse): IV fluids NOW - Ringer's lactate fast; this is the ambulance moment
Step 3: Hospital care for the severe
Rapid IV rehydration first, then transition to ORS as the patient stabilizes. Antibiotics (doxycycline or azithromycin) for severe cases shorten the illness. Zinc for children. Glucose watch in children. With this care, even cholera gravis fatality falls below 1%.
Step 4: Recovery feeding and home completion
Continue ORS until stools normalize; eat light and protein-forward; return to full diet over the week. The patient stops being contagious as shedding ends (antibiotics shorten this); household hygiene holds for a few extra days.
The classical layer
Classical atisara recovery feeding - rice gruels, buttermilk, staged light diet - fits the recovery phase naturally. During the purging and dehydration stages, modern fluids own the case completely.
The two rules that override everything
- Fluids begin at the first stool, not at the diagnosis.
- Dehydration signs mean hospital - cholera's timeline forgives no waiting.
One-line summary
ORS from the first stool, facility care the moment dehydration signs appear, IV fluids and antibiotics for the severe, full recovery the rule - cholera treatment is fast fluids, correctly escalated.
