Complications and Risks of Cholera

Reviewed by medical experts · For informational purposes only

Introductory summary

Shock, salt crashes, kidney failure, child hypoglycemia - all downstream of untreated fluid loss, all preventable.

Cholera's complications all flow from one river: fluid and salt loss. Stopped early, there are none; allowed to run, they cascade fast. Here is the risk map.

Dehydration and shock (the main event)

  • Rapid fluid loss collapses circulation: falling blood pressure, racing then fading pulse, cold extremities, clouded consciousness
  • Hypovolemic shock is cholera's killer - and it is prevented and reversed by fluids more reliably than almost any complication in medicine

Electrolyte crash

  • Potassium loss: severe weakness, muscle cramps, dangerous heart-rhythm disturbances
  • Sodium imbalance: confusion, seizures in extremes
  • Acidosis: the blood turns acidic from lost bicarbonate - deep rapid breathing, worsening shock
  • ORS contains these salts precisely because losing them is the danger

Kidney failure

Prolonged low blood pressure starves the kidneys: urine stops, toxins accumulate. Acute kidney injury is the main organ complication - usually reversible with fluids, occasionally needing dialysis in neglected cases.

Low blood sugar (children)

Purging children can crash their glucose: lethargy, staring, seizures. Facilities check and correct sugar alongside fluids - a reason children with severe watery diarrhea belong in care early.

In pregnancy

Severe dehydration endangers both mother and fetus - fetal loss risk rises with maternal shock. Pregnant patients with profuse watery diarrhea: hospital immediately, no home trial.

After recovery

Treated cholera leaves no lasting damage: the gut heals fully, immunity to the same strain lasts a while, and there is no chronic form. The complications belong to the untreated hours, not to the recovered patient.

One-line summary

Shock, salt crashes, kidney failure and child hypoglycemia - all downstream of untreated fluid loss, all preventable by the ORS-IV-fluid escalation started in time.
