Stages of Cholera

Cholera is a disease of hours, not weeks. Its stages measure the body's fluid tank emptying - and each stage has its own urgency and its own correct response.

Stage 1: Incubation (hours to 5 days, typically 1-3)

The swallowed bacteria establish in the small intestine. No symptoms. A person who drank suspect water at a wedding or flood camp is in this window.

Stage 2: Onset (first hours)

Sudden opening: watery diarrhea begins, often with vomiting, usually without warning, pain or fever. Start ORS immediately - do not wait to see the pattern.

Stage 3: Purging and dehydration (hours to 1-2 days)

Output peaks - up to a liter an hour in severe cases. The dehydration ladder climbs: thirst, dry mouth, sunken eyes, poor skin pinch, scant urine, racing pulse, muscle cramps from lost salts. This is the make-or-break window: aggressive ORS holds mild-moderate cases; IV fluids rescue severe ones. Every hour of delay steepens the slope.

Stage 4: Collapse (the danger floor)

Untreated severe loss leads to shock: cold extremities, barely measurable blood pressure, no urine, clouded consciousness, and organ failure. Kidney failure from prolonged low pressure is the main internal complication. This stage is why cholera is an ambulance illness, not a wait-and-watch one.

Stage 5: Recovery (days 2-7)

With fluids sustained, the bacteria clear and output slows over 2-5 days. Appetite and energy return; salts and sugar rebalance. Antibiotics, when used, shorten this shedding phase by a couple of days. Full recovery without lasting damage is the rule in treated patients.
