Symptoms of Cholera

Reviewed by medical experts · For informational purposes only

Introductory summary

Painless profuse rice-water diarrhea, vomiting, little fever - and a dehydration ladder from thirst to collapse within hours.

Cholera's symptoms are deceptively simple: an enormous amount of watery diarrhea, without pain and usually without fever. Recognizing the pattern - and the dehydration it produces - is what saves the patient.

The diarrhea itself

  • Sudden onset of frequent, large-volume watery stools
  • Classically "rice-water": pale, cloudy, flecked, with a mild fishy smell - not bloody, not mucus-filled
  • Painless: no cramps of dysentery, usually no fever or only a mild one
  • Vomiting, often early, worsening the fluid loss
  • Output can reach a liter an hour in severe cases - speed is the disease's whole strategy

The dehydration ladder (watch this, not the stool count)

  • Early: intense thirst, dry mouth, reduced urine
  • Moderate: sunken eyes, dry skin that stays tented when pinched, fast pulse, weakness, irritability or lethargy
  • Severe: barely any urine, cold wrinkled hands ("washerwoman's hands"), muscle cramps, low blood pressure, drowsiness or unconsciousness - collapse can follow within hours

In children

Faster and more dangerous: children dehydrate quicker and can also drop blood sugar (look for lethargy, staring, seizures). Any child with profuse watery diarrhea and weakness needs fluids and medical care immediately.

Mild and silent cases

Most infections are actually mild or symptom-free - ordinary loose motions or nothing at all. These hidden carriers still shed bacteria and seed outbreaks, which is why water hygiene matters even when nobody "looks sick".

The emergency line

Rice-water stools + sunken eyes + weakness = hospital now, ORS on the way. Cholera's timeline is hours, not days.

One-line summary

Painless profuse rice-water diarrhea with vomiting and little fever, climbing a dehydration ladder from thirst to collapse within hours - fluids immediately, hospital early.
