Stages of Dengue

Introductory summary
Dengue moves through three stages: the febrile phase, the critical phase when fever falls but danger peaks, and the recovery phase.
Dengue runs a three-act course over about ten days, and the plot twist is built in: the middle act - when the fever breaks - is the dangerous one. Knowing the stages tells you when to relax and when to watch hardest.
Stage 1: Febrile phase (days 1-4)
- Sudden high fever, severe body ache, headache with pain behind the eyes, nausea, flushed face
- What to do: NS1 test to confirm, paracetamol only for fever, steady fluids, rest, and start the platelet count tracking
- Most patients feel terrible but are in no danger yet
Stage 2: Critical phase (roughly days 3-7 - the trap)
- The fever FALLS - and this is exactly when plasma can start leaking from blood vessels in the unlucky few
- The platelet count typically hits its lowest here
- Watch like a hawk for warning signs: abdominal pain, persistent vomiting, bleeding (gums, black stools), lethargy or restlessness, cold hands and feet
- Any warning sign = hospital - this phase is when IV fluids given at the right time are life-saving, and given at home they simply do not exist
- Most patients sail through with nothing worse - but nobody knows in advance who will not, which is why the watching matters
Stage 3: Recovery (days 7-10 onward)
- Appetite returns, platelets climb, the rash may itch as it fades
- Weakness, low mood and tiredness can linger for weeks - normal, gradually improving
- What to do: continue fluids and good food, ease back to normal activity over 1-2 weeks, finish the follow-up blood tests the doctor asks for
One-line summary
Dengue's stages are the febrile days (test, paracetamol, fluids), the critical days 3-7 window after fever falls (watch for abdominal pain, vomiting, bleeding, lethargy - hospital if any appear), and a recovery week or two of rebuilding - the trap being that the dangerous stage looks like improvement.
