Stages of reflux disease

Introductory summary

How reflux disease progresses and why early control matters.

Reflux disease can progress slowly over years if it is not controlled. There is no single official staging system, but doctors often describe a progression from mild symptoms to complications. Understanding this helps explain why steady control matters.

Early or mild stage Heartburn occurs occasionally, usually after large or trigger meals. The oesophagus lining looks normal. Lifestyle changes alone often settle it.

Persistent stage Symptoms occur two or more times a week, may wake you at night and need regular antacids or prescribed medicine. Some people have a cough or hoarseness.

Oesophagitis stage Repeated acid exposure inflames the lining. Endoscopy shows redness or erosions, graded from mild to severe (for example the Los Angeles grades A to D).

Complication stage

  • Narrowing of the food pipe (stricture), causing food to stick.
  • Bleeding from ulcers in the oesophagus.
  • Barrett's oesophagus, a change in the cells lining the lower food pipe that needs regular surveillance.
  • Rarely, oesophageal cancer.

What this means in practice Many people stay at the early stages for life. Progression is more likely with long-standing untreated symptoms, obesity, smoking and hiatus hernia. Treating reflux early lowers the chance of complications.

Ayurvedic view (interpretive) Ayurveda describes disease as moving from imbalance to deeper tissue involvement over time, which loosely parallels the idea that untreated acidity can progress. This is a conceptual comparison only.

Remember Alarm signs such as trouble swallowing, vomiting blood, black stools or weight loss need prompt medical review at any stage.
