Introductory summary
Varicose veins range from cosmetic spider veins to bulging trunk varicosities to advanced disease with skin changes and ulcers; doctors grade severity to decide between stockings and procedures.
Types of varicose veins
By appearance and size:
- Spider veins (telangiectasia): fine red, blue or purple webs of tiny surface vessels. Cosmetic mainly; treated with sclerotherapy or laser if they bother you. They do not become big varicose veins - they are their own mild category.
- Reticular veins: slightly larger blue-green veins under the skin, feeding the spider webs; usually cosmetic plus mild ache.
- Trunk varicose veins: the true bulging, rope-like veins of the great or small saphenous system - the type that causes heaviness, swelling and, eventually, skin damage.
By origin:
- Primary varicose veins: valve failure arising on its own (family tendency plus life factors) - the vast majority.
- Secondary varicose veins: superficial veins overloaded because the deep system was damaged - usually by a previous deep vein clot (DVT). These need more careful evaluation before any procedure, since the superficial veins may be doing compensatory work.
By severity - the CEAP grading doctors use (simplified):
- C1: spider/reticular veins only
- C2: visible varicose veins
- C3: plus ankle swelling (oedema)
- C4: plus skin changes - darkening, eczema, hardening
- C5: a healed venous ulcer
- C6: an active venous ulcer
This grading is practical, not academic: C1-C2 usually means stockings, lifestyle and optional cosmetic procedures; C3-C4 means a proper venous Doppler ultrasound and a discussion about closing the faulty vein (laser/radiofrequency/sclerotherapy); C5-C6 needs specialist wound and vein care.
A related type to know: varicocele - varicose veins of the scrotum in men, a different condition entirely (affects fertility discussions); and haemorrhoids are, mechanically, varicose veins of the anal region - same valve-pressure logic, different address.
Remember: spiders are cosmetic, trunks cause symptoms, skin changes and ulcers mean advanced disease needing specialist care - and secondary (post-DVT) veins need careful imaging before treatment.
Ayurvedic view (interpretive)
Classical descriptions do not grade vein disease; the modern CEAP ladder exists because treatment intensity climbs with grade. Typing is what converts "nason ki problem" into a plan.
