Causes of Body Ache

Introductory summary
What causes body ache: viral illness, overexertion, poor sleep, vitamin D and B12 deficiency, thyroid trouble - and the must-not-miss causes like rhabdomyolysis and dengue.
The list of things that can make your whole body ache is long - but the everyday causes account for almost all of it. Here is the map, from the common to the must-not-miss.
The everyday causes
- Viral infections: the classic cause. Flu, common colds, dengue, chikungunya, COVID - immune signalling chemicals produce the all-over ache, usually with fever, sore throat or runny nose
- Overexertion: unaccustomed exercise, heavy lifting, a long day of physical work. Delayed soreness peaks 24-72 hours later and resolves on its own
- Poor sleep: even a few nights of short or broken sleep measurably lowers pain thresholds
- Dehydration and heat: especially in Indian summers, with cramps and heaviness
- Stress and anxiety: sustained muscle tension, especially neck, shoulders and back
- Prolonged sitting or bad posture: the desk-work ache
Deficiencies and medical causes
- Vitamin D deficiency: extremely common in India; causes diffuse aching and fatigue
- Vitamin B12 deficiency, low iron, thyroid underactivity: each can present as persistent ache with tiredness
- Fibromyalgia: chronic widespread pain with unrefreshing sleep and fatigue, diagnosed after excluding other causes
- Infections beyond the usual: dengue and chikungunya (seasonal, with fever), typhoid, and others
- Medication side effects: statins are the classic example
- Inflammatory conditions: polymyalgia rheumatica in people over 50 (shoulder and hip girdle aching with morning stiffness), rheumatoid arthritis and others
- Rhabdomyolysis: muscle breakdown after extreme exertion, heat stroke or certain drugs - dark, cola-coloured urine is the warning sign and an emergency
The MedlinePlus overview of muscle aches covers this same spectrum well.
The classical view
The classical causes of Angamarda read like a commentary on the modern list: overexertion beyond one's capacity (Vyayama and load-carrying), Vata-provoking cold and dry exposure, Ama from weak digestion, and - centuries before sleep science - suppression of sleep and hunger as direct causes of body pain (Ashtanga Hridaya, Sutra Sthana 4). The framework is traditional and interpretive; persistent or unexplained ache still belongs to blood tests and a doctor, not to theory.
