Types of Dry Eye

Introductory summary
Evaporative vs aqueous-deficient dry eye - blocked oil glands vs too few tears - why the type decides the treatment.
Dry eye is not one problem but two, and most people have a mix. Knowing the type matters because the treatments differ.
Evaporative dry eye - the common majority. The eyes make enough watery tears, but the tears evaporate too fast. The usual reason is meibomian gland dysfunction (MGD): the rows of tiny oil glands along the eyelid margins get blocked or sluggish, so the tear film loses the thin oily coat that normally seals moisture in. Think of it as a pot without a lid. Contributing factors include screen-heavy days (incomplete blinking skips the oil-spreading sweep), eyelid inflammation and dandruff-like lid margin disease (blepharitis), contact lens wear, and living in air conditioning. The signature treatment is heat and lid care - warm compresses and lid massage - because the goal is to get the oil flowing again.
Aqueous-deficient dry eye - the less common type. The tear glands genuinely underproduce the watery layer. This type is linked to ageing, menopause, certain medicines (antihistamines, isotretinoin, some antidepressants), damage from eye surgery, and - importantly - autoimmune conditions, above all Sjogren's syndrome. When dryness is severe, constant, and paired with a dry mouth, this is the type doctors look harder for, because it can signal a whole-body condition that needs its own care.
Mixed dry eye. Most real-world patients sit here: some gland blockage, some underproduction, and a lifestyle full of screens and AC air working against both.
A word on the look-alikes: allergic eye disease, conjunctivitis and blepharitis can all mimic dry eye, and they are treated differently. Allergy itches; dry eye burns and feels gritty. If itching dominates, the answer is usually allergy, not tears.
An eye doctor can sort the type in one visit - which is worth doing if symptoms persist, because the right type means the right treatment the first time.
