How to treat or cure otitis media

Reviewed by medical experts · For informational purposes only

Introductory summary

How otitis media is treated, from watchful waiting to antibiotics, grommets and surgery.

Most ear infections get better, but the right care depends on age, severity and type.

Acute otitis media

  • Pain relief is the first step for everyone
  • Watchful waiting for 48 to 72 hours is an option for healthy children over 6 months with mild illness in one ear, provided a follow-up plan is clear
  • Antibiotics are given for babies under 6 months, severe illness, high fever, both ears affected, ear discharge, or if symptoms do not improve
  • Review if there is no improvement in 2 to 3 days

Otitis media with effusion

Often watched for about 3 months. A hearing test is advised if fluid persists. Grommets may be offered if hearing is affected, speech is delayed or fluid keeps returning.

Chronic suppurative otitis media

  • Aural toilet: a doctor cleans the ear
  • Antibiotic drops prescribed for discharge
  • Keep the ear dry
  • Surgery (tympanoplasty or mastoidectomy) to repair the drum or remove disease, including cholesteatoma

Ayurvedic support

Some families add Ayurvedic care for general well-being. Evidence is limited and ear oils and drops can be unsafe with a perforated eardrum. Use only after a doctor has checked the ear, and never in babies without advice.

See a doctor if there is swelling behind the ear, facial weakness, a stiff neck, or hearing loss.

When to get help

See a doctor or ENT clinic if ear pain, discharge or hearing change does not settle in a couple of days, if infections keep coming back, or if you are unsure about any medicine or drop. Early assessment protects hearing and helps avoid unnecessary treatment.

Remember

Treat the cause and protect hearing. Early medical care prevents complications.
