Causes of Bloating

Introductory summary
What causes bloating: swallowed air, gas-forming foods, lactose intolerance, constipation, IBS and SIBO - plus the persistent-bloating warning signs that need a doctor.
Most bloating comes from gas - too much of it made in the gut, or the gut handling a normal amount badly. Occasionally it signals something that needs a doctor. Sorting the everyday causes from the warning ones is the whole game.
Swallowed air
Some bloating starts at the top: eating fast, talking while eating, chewing gum, sucking hard candy, drinking through a straw, fizzy drinks, and smoking all send extra air into the stomach. Much of it comes back up as belching; some travels downward.
Gas made in the gut
Bacteria in the colon ferment undigested carbohydrate and release gas. The usual fuel:
- Beans, lentils and chickpeas (famous for a reason)
- Cabbage, cauliflower, broccoli, onion and garlic
- Wheat and other high-FODMAP foods in sensitive people
- Milk and dairy in people with lactose intolerance - very common in Indian adults
- Sugar-free products containing sorbitol and other sweeteners
- Large, heavy meals that overwhelm digestion
Common medical causes
- Constipation: a loaded colon ferments longer and blocks gas from moving out
- Irritable bowel syndrome (IBS): a sensitive gut-brain axis makes normal gas feel painful and visible
- SIBO (small intestinal bacterial overgrowth): bacteria fermenting in the wrong part of the gut
- Celiac disease: an immune reaction to wheat gluten; worth testing when bloating pairs with loose stools, anemia or weight loss
- Gastroparesis (slow stomach emptying), especially in long-standing diabetes
- Hormonal shifts: many women bloat before periods and in early pregnancy
Causes that must not be missed
Persistent or progressive bloating - most days for three weeks or more - deserves a doctor, not another home remedy, especially with weight loss, poor appetite, blood in stool, anemia, fever, or severe pain. In women, persistent bloating is one of the recognized warning signs of ovarian cancer and is specifically flagged on the NHS bloating page. Sudden severe distension with vomiting and no passage of stool or gas is an emergency (possible bowel obstruction).
The classical view
Ayurveda called bloating Adhmana - Vata accumulating in the gut - and blamed weak digestive fire (Agnimandya) producing Ama (undigested residue), plus dietary errors: overeating, eating before the previous meal has digested (Adhyashana), irregular meals, and cold, dry, heavy food. Charaka and Sushruta also listed suppression of natural urges (flatus, stool, belching) as a direct cause. Much of this maps surprisingly well onto the modern list above; it remains an interpretive framework, not a substitute for evaluating red flags.
