Symptoms of Impetigo

Introductory summary

Impetigo symptoms: red sores with honey-coloured crusts, bullous blisters, and signs that need a doctor.

Impetigo usually starts as small red spots that turn into blisters or sores, most often around the nose and mouth, but also on the hands, arms and legs.

Non-bullous impetigo (most common)

  • Small red sores that quickly burst and ooze
  • A typical honey-coloured or golden crust
  • Itching and mild soreness
  • Sores that spread to nearby skin by scratching
  • Swollen glands nearby in some people

Bullous impetigo

  • Larger, fluid-filled blisters, mainly on the trunk, arms or in skin folds
  • The blisters burst, leaving a thin, yellow-brown crust

Ecthyma (deeper form)

  • Painful sores with a thick crust that heal slowly, often with a scar

Usually not present

High fever and severe illness are uncommon. They suggest a spreading infection.

See a doctor or paediatrician if

  • A baby or newborn has sores or blisters
  • The rash spreads fast or covers a large area
  • Fever, pain, warmth, swelling or red streaks appear
  • Sores do not improve after 2 to 3 days of treatment
  • The rash is near the eyes
  • Tea-coloured or reduced urine, or puffy face and eyes, appear a few weeks after infection
  • The person has diabetes, eczema or weak immunity

Key point

Honey-coloured crusts on red sores, especially near the nose or mouth of a child, are the classic sign. Early treatment shortens the illness and limits spread.

Before your appointment

Note when the sores began, where they started, whether they ooze or crust, and whether anyone at home or school has similar sores. List any fever, itching, eczema, scabies, diabetes, allergies and medicines tried so far. Take a photo of the rash. Ask about the antibiotic, when it is safe to return to school and the warning signs that mean you should call 112 or 102.
