Introductory summary

Medicines for PCOS: contraceptive pills, metformin, anti-androgens and fertility drugs, with safety points.

Medicines for PCOS depend on your symptoms and whether you want to conceive. A gynaecologist or endocrinologist should prescribe them.

For irregular periods and protection of the womb lining

  • Combined oral contraceptive pills: regulate periods, lower androgens and protect the lining. Not suitable for everyone, for example those with clot risk, some migraine types or who smoke.
  • Progestin courses to bring on a period every few months

For excess hair and acne

  • Anti-androgens such as spironolactone, usually with contraception, as they can harm a developing baby
  • Topical treatments for acne, and procedures such as laser hair removal

For insulin resistance and weight

  • Metformin: may improve cycles, blood sugar and weight in some women. Common side effects are nausea and loose stools.
  • Other options: inositol, GLP-1 based medicines and weight management drugs in selected cases, as advised by a specialist

For fertility

  • Letrozole is the usual first medicine to trigger ovulation
  • Clomiphene, gonadotropin injections, ovarian drilling, or IVF if needed

Ayurvedic and herbal products

Many products are sold for PCOS. Evidence is limited, and some interact with hormonal and diabetes medicines. Avoid them when trying to conceive or in pregnancy unless your doctor agrees.

Safety points

  • Do not take anti-androgens or herbal hormone products in pregnancy
  • Report severe headache, leg swelling or chest pain on contraceptive pills
  • Do not buy hormones without a prescription

See a doctor if you have side effects, or you want to plan a pregnancy.

Remember

Treatment is tailored to your goals. Share them clearly with your doctor.
