How to treat or cure sinusitis

Introductory summary
How sinusitis is treated, and when antibiotics are needed.
Most acute sinusitis is caused by a virus and gets better in 7 to 10 days with self-care. Antibiotics help only for bacterial infection, and are not needed in most cases. Chronic sinusitis often needs longer treatment and a plan to control inflammation.
Self-care first Saline rinse, steam, warm compresses, fluids, rest and simple pain relief such as paracetamol. See the home remedies section for details.
Medicines a doctor may advise
- Steroid nasal sprays (such as fluticasone or mometasone) reduce swelling and are the main treatment for chronic sinusitis and allergy-related sinusitis. They take days to work and need regular use.
- Short-term decongestant nasal sprays such as oxymetazoline can help, but should not be used for more than three days, since they cause rebound congestion.
- Oral antihistamines if allergy is a cause.
- Antibiotics only when bacterial infection is likely, such as symptoms for more than 10 days, worsening after initial improvement, or severe illness. A doctor chooses the drug and the course length.
- Oral steroids for short periods in severe chronic sinusitis or with polyps.
For chronic or recurring sinusitis
- Treat allergies and avoid triggers.
- A nose and throat (ENT) specialist may do an endoscopy or a CT scan.
- Surgery, such as functional endoscopic sinus surgery, can open blocked drainage pathways, remove polyps or correct a deviated septum when medicine has failed.
Is there a cure? Acute sinusitis usually clears completely. Chronic sinusitis can often be controlled, though it may flare again and need long-term care.
Ayurvedic view (interpretive) Nasya, steam and herbs may offer comfort, but evidence is limited. They should be an adjunct to medical care.
When to get urgent help Swelling around the eyes, vision change, severe headache, stiff neck, confusion or high persistent fever need urgent medical care.
Remember See a doctor if symptoms last more than 10 days, worsen, or keep returning.
