Stages of ulcer

Introductory summary
An ulcer moves from surface inflammation to an open sore, then to healing with treatment - or, untreated, to bleeding, perforation or narrowing; early treatment stops this progression.
Stages of ulcer
Doctors do not stage ulcers like cancer, but the disease does follow a recognisable progression - and treatment at the early steps prevents the dangerous late ones.
Stage 1 - Irritation and inflammation (gastritis/duodenitis): the lining is inflamed but intact. Symptoms are mild - acidity, bloating, vague burning after irritant foods or painkillers. Reversible with acid control and removing the cause.
Stage 2 - Erosion: small superficial breaks appear in the lining, not yet through its full thickness. Burning pain becomes more regular, empty-stomach pain appears, antacids give shorter relief.
Stage 3 - Frank ulcer: a full-thickness open sore. Now the pattern is classic - regular burning pain on an empty stomach or at night, relief with food or antacid. At this stage H. pylori testing and proper treatment are essential; ulcers at this depth can reach blood vessels.
Stage 4 - Complications (the dangerous stage):
- Bleeding - the ulcer erodes into a blood vessel: vomiting blood or black stools, weakness, dizziness. Emergency.
- Perforation - the ulcer eats through the entire wall: sudden severe abdominal pain, rigid abdomen. Surgical emergency, call 112.
- Penetration - the ulcer bores into a neighbouring organ like the pancreas: deep, constant back pain.
- Obstruction - scarring near the stomach outlet narrows it: persistent vomiting of stale food, weight loss.
Stage 5 - Healing (with proper treatment): with H. pylori eradication and 4-8 weeks of acid suppression, most ulcers heal completely. A gastric ulcer should be confirmed healed - and biopsied if it does not - to rule out cancer.
Remember: irritation → erosion → ulcer → complications is the untreated path; treatment at any earlier stage blocks the emergencies. Recurring empty-stomach pain is the moment to act, not after blood appears.
Ayurvedic view (interpretive)
The progression from Amlapitta's surface burning to deeper vrana (wound) loosely parallels erosion-to-ulcer. The practical point matches: treat early, because the later stages belong to surgeons, not remedies.
