Types of vaginal infection

Introductory summary

The main types are candida (fungal, itchy curd discharge), bacterial vaginosis (fishy grey discharge), trichomoniasis (STI, partner needs treatment) - and not every discharge is an infection at all.

Types of vaginal infection

"White discharge" is one complaint with several different diseases behind it - and each has its own medicine. This is why the chemist's single "infection kit" so often fails.

1. Vulvovaginal candidiasis (fungal/yeast infection): the commonest. Candida yeast overgrowth. Intense itching, thick white curd-like discharge, redness. Triggers: antibiotics, diabetes, pregnancy, synthetic tight clothing. Treated with antifungals (vaginal tablets/creams or a single oral dose). Recurrent candida (4+ episodes a year) needs a diabetes check and longer suppressive treatment.

2. Bacterial vaginosis (BV): a balance-shift, not an invasion - protective lactobacilli drop, other bacteria overgrow. Thin grey discharge, fishy smell, little itch. Treated with specific antibiotics (metronidazole or clindamycin). It recurs often; it is associated with sexual activity but is not classified as an STI - partners usually do not need treatment.

3. Trichomoniasis: a true sexually transmitted infection (a parasite). Yellow-green frothy discharge, smell, itch, discomfort. Treated with a specific oral antibiotic - and the partner must be treated simultaneously, or the infection ping-pongs back.

4. Other causes of discharge/symptoms worth knowing:

  • Aerobic vaginitis and other rarer imbalances - need a swab test to identify
  • STIs beyond trichomoniasis: chlamydia and gonorrhoea often cause few symptoms but can silently damage the tubes - persistent discharge with pelvic pain or after a new partner deserves testing
  • Atrophic vaginitis (post-menopause): dryness, irritation and discharge from low estrogen - treated completely differently (estrogen-based)
  • Non-infective causes: allergy to a wash or condom material, a forgotten tampon (causes very foul discharge), and dermatological conditions

5. Not an infection at all: physiological discharge - the normal clear/milky discharge that varies with the cycle, increases before ovulation and in pregnancy. This needs nothing, and treating it with unnecessary creams creates the imbalance that causes real infections.

Remember: candida itches, BV smells, trichomoniasis is sexually transmitted - and normal discharge needs no treatment at all. A correct type means a correct medicine; a swab test settles any doubt.

Ayurvedic view (interpretive)

Classical yoni-vyapad categories group many discharge conditions. Modern typing by organism and mechanism is what makes the treatment specific - the two systems agree that one remedy does not fit all discharge.
