Stages of Goiter
Reviewed by medical experts · For informational purposes only
Introductory summary
Goiter is graded by size from palpable to visible, and may become nodular or press on the windpipe over time.
Goiter has no single official staging system, but doctors describe its size and how it behaves over time. Most remain stable or grow slowly for years.
Grade 0: No goiter
The thyroid is neither visible nor felt.
Grade 1: Palpable goiter
The thyroid can be felt on examination but is not visible when the neck is in normal position.
Grade 2: Visible goiter
The swelling is visible when the neck is in normal position.
Nodular change
Over many years, a long-standing diffuse goiter may develop nodules, and some nodules become overactive.
Pressure effects
A large goiter can press on the windpipe or food pipe and cause cough, noisy breathing or trouble swallowing.
Change in function
Thyroid hormone levels can become low or high at any stage and need checking.
What can change the course
- Correcting iodine deficiency can shrink a diffuse goiter
- Treating autoimmune thyroid disease
- Medicines or surgery when it is large or overactive
Key point
Regular follow-up with blood tests and ultrasound matters, since nodules can change. Difficulty breathing or swallowing, or rapid growth, needs urgent care. Call 112 or 102 if breathing is hard.
Before your appointment
Carry earlier thyroid reports, a list of medicines and supplements, and notes on swelling, weight change, heartbeat, mood and periods. Tell your doctor about any family history of thyroid disease and whether you are pregnant or planning pregnancy. This helps pick the right tests and treatment.
